6. Cơ cấu tổ chức

6.1. Sơ lược quá trình phát triển

Quá trình thành lập và phát triển khoa Tai mũi họng gắn liền với sự hình thành và phát triển của Bệnh viện. Tháng 7 năm 1951, Bệnh viện Trung ương Thái Nguyên được thành lập tại Đồng Hỷ - Thái Nguyên lấy tên là Bệnh viện Liên khu Việt Bắc. Thời gian đầu thành lập, hoạt động chuyên môn và sinh hoạt Chi bộ của khoa Tai mũi họng nằm trong hoạt động của Liên khoa Mắt - Tai mũi họng - Răng hàm mặt.

Năm 1962, khoa Tai mũi họng được tách riêng từ Liên chuyên khoa, các đảng viên khoa Tai mũi họng sinh hoạt chi bộ cùng Chi bộ Liên chuyên khoa. Năm 2001, Chi bộ Tai mũi họng được tách riêng và sinh hoạt độc lập.

Là một chuyên khoa sâu thuộc hệ ngoại, khoa Tai mũi họng có nhiệm vụ thăm khám, phát hiện và điều trị nội khoa, phẫu thuật các bệnh lý thuộc chuyên ngành Tai mũi họng phục vụ nhân dân Tỉnh Thái Nguyên, các tỉnh miền núi phía Bắc và các tỉnh lân cận.

Trải qua hơn 70 năm hình thành và phát triển, khoa Tai mũi họng dưới sự lãnh đạo của Đảng bộ, Ban Giám đốc, sự tạo điều kiện của các Phòng chức năng, tập thể khoa đã đang và từng bước lớn mạnh theo sự phát triển chung của Bệnh viện trong thời kỳ mới.

7. Hoạt động chuyên môn

Là 1 chuyên khoa lẻ hệ ngoại thuộc Bệnh viện tuyến trung ương, nằm ở trung tâm của các tỉnh miền núi phía bắc, khoa Tai Mũi Họngngoài công tác khám chữa bệnh trên địa bàn tỉnh, khoa còn là địa chỉ tin cậy, là tuyến trên của các bệnh viện tỉnh lân cận, các tỉnh miền núi phía Bắc. Khoa Tai Mũi Họng luôn định hướng phát triển theo từng thời kì, thường xuyên cử cán bộ học tập nâng cao chuyên môn và cập nhật kiến thức khoa học, kĩ thuật mới, trang thiết bị hiện đại, phát triển cơ sở hạ tầng, cùng với thái độ phục vụ, chăm sóc người bệnh, phối hợp nhịp nhàng giữa nhân viên y tế với người bệnh, giữa bác sĩ và điều dưỡng.

Cụ thể:

-Tổng số giường kế hoạch: 35, giường thực kê: 54

- Số buồng bệnh: 08

- Số lượt khám/ngày tại phòng khám: 80 đến 100 bệnh nhân/ngày.

- Giải quyết các cấp cứu Tai mũi Họng như: Cầm máu mũi, mở khí quản, dị vật tai, mũi họng, thực quản, các chấn thương vùng tai mũi họng và đầu cổ…

- Những kĩ thuật thăm khám và làm thủ thuật:

+ Khám nội soi Tai Mũi Họng

+ Đo sức nghe, đo nhĩ lượng, phản xạ cơ bàn đạp, đo OAE, thăm dò chức năng tai …

+ Làm thủ thuật chuyên khoa: Chọc rửa xoang, hút rửa mũi xoang, khí dung mũi họng, làm thuốc tai, chích rạch màng nhĩ dẫn lưu, chích rạch áp xe vùng Tai Mũi Họngvà đầu cổ, sinh thiết các khối u vùng Tai Mũi Họngvà đầu cổ, làm Proetz, bơm hơi vòi nhĩ, bơm thuốc thanh quản, …

+ Các phẫu thuật chính triển khai tại khoa: hiện tại khoa đã áp dụng các kỹ thuật hiện đại trong các phẫu thuật cấp cứu và thường quy tại khoa cụ thể như:

* Lĩnh vực phẫu thuật tai

- Phẫu thuật cắt đường rò luân nhĩ.

- Phẫu thuật chỉnh hình ống tai ngoài.

- Phẫu thuật đặt ống thông khí gây mê, gây tê.

- Phẫu thuật nội soi vá nhĩ đơn thuần, vá nhĩ dưới kính hiển vi.

- Phẫu thuật sào bào, thượng nhĩ.

- Phẫu thuật chỉnh hình tai giữa.

- Phẫu thuât tiệt căn xương chũm.

- Phẫu thuật điều trị biến chứng nội sọ do tai.

- Phẫu thuật giảm áp dây VII trong chấn thương xương thái dương.

* Lĩnh vực phẫu thuật mũi xoang

· Phẫu thuật Caldwell Luc lấy u nang sàn mũi…..

· Phẫu thuật lấy các khối u vùng mũi, xoang(u xoang trán, xoang bướm..). Phối hợp với ngoại thần kinh phẫu thuật lấy u tuyến yên qua đường nội soi

· Phẫu thuật nội soi mũi chức năng mũi xoang.

· Phẫu thuật nội soi chỉnh hình vách ngăn mũi.

· Phẫu thuật nội soi chỉnh hình cuốn mũi giữa, cuốn dưới bằng đông điện, Lazer, Plasma..

· Phẫu thuật nâng xương chính mũi gây tê, gây mê..

· Phẫu thuật nội soi cầm máu mũi (cắt đốt động mạch bướm khẩu cái, phối hợp với khoa CĐHA nút mạch cầm máu mũi…)

· Phẫu thuật Sistrunk cắt u nang giáp móng.

· Phẫu thuật điều trị ung thư tuyến giáp, u tuyến giáp lành tính sử dụng dao Ligasure: cắt thùy tuyến giáp, cắt gần toàn bộ tuyến giáp, cắt toàn bộ tuyến giáp, nạo vét hạch cổ.

· Phẫu thuật u tuyến mang tai: cắt thùy nông, cắt thùy sâu, cắt toàn bộ tuyến mang tai.

+ Điều trị bảo tồn, nội khoa:

· Các bệnh lý viêm nhiễm cấp, bán cấp, mạn tính: viêm tai, viêm mũi xoang, viêm họng, viêm thanh quản…

· Một số tổn thương chức năng: Điếc đột ngột, rối loạn tuần hoàn tai trong…

8. Thành tích nổi bật

· Khoa TMH đã thực hiện được 215/287 đạt tỷ lệ 77,25% các kỹ thuật chuyên môn đã thực hiện được theo phân tuyến kỹ thuật theo thông tư 43/2013/TT-BYT ngày 11 tháng 12 năm 2013 của Bộ trưởng Bộ Y Tế về “Phân tuyến kỹ thuật và danh mục kỹ thuật trong khám chữa bệnh chuyên khoa TMH ”

· Khoa hoàn thành ít nhất 02 đề tài nghiên cứu khoa học cấp cơ sở/ năm của bác sĩ và điều dưỡng khoa.

· Năm 2023, Hoàn thành xuất sắc dự án khoa học cấp Tỉnh “Ứng dụng dao Ligasure trong phẫu thuật ung thư tuyến giáp tại Thái Nguyên” đưa kỹ thuật này vào ứng dụng hiệu quả trong thực tế lâm sàng

· Khoa phối hợp với các khoa/phòng/ trung tâm liên quan thực hiện đề tài cấp Nhà nước “Nghiên cứu một số yếu tố nguy cơ và đặc điểm hệ gen mã hóa ở bệnh nhân khiếm thính bẩm sinh tại Thái Nguyên” dưới sự hướng dẫn chỉ đạo của Ban Giám đốc Bệnh viện, Ban Chủ nhiệm đề tài.

· Cán bộ khoa tham gia công tác đào tạo các khóa đào tạo ngắn hạn chuyên ngành Tai Mũi Họng do Bệnh viện tổ chức; tham gia giảng dạy sinh viên, học viên chuyên khoa cấp I, II Trường Đại học Y - Dược Thái Nguyên về chuyên ngành Tai Mũi Họng.

· Khoa TMH góp phần trong việc thành lập Hội tai mũi họng tỉnh Thái Nguyên Hội thành lập từ 02/6/2015 theo quyết định số 605/QĐ- YBND ngày 16/3/2015, từ đó đến này hội không ngừng lớn mạnh về cả quy mô và số lượng.

10. Huấn luyện đào tạo

- Khoa Tai Mũi Họngkết hợp với Bộ môn Tai Mũi HọngTrường Đại học Y Dược Thái nguyên là nơi thực hành đào tạo các học viên chuyên khoa cấp 1, chuyên khoa cấp 2 tai mũi họng, bác sĩ nội trú tai mũi họng, thạc sĩ và chuyên khoa 1 các chuyên ngành khác như Nhi, Răng hàm mặt…

- Tham gia chỉ đạo tuyến, chuyển giao kĩ thuật cho các bệnh viện tuyến dưới trong địa bàn tỉnh và các tỉnh lân cận…

- Đào tạo, nâng cao trình độ chuyên khoa cho nhiều y, bác sĩ tuyến dưới đến học tập

- Đào tạo các lớp chứng chỉ nội soi tai mũi họng, các lớp phẫu thuật ngắn hạn, dài hạn như phẫu thuật cắt Amidan, phẫu thuật nội soi nạo VA, phẫu thuật mũi xoang….

11. Phương hướng, định hướng phát triển

Mục tiêu tổng quát: Nâng cao chất lượng điều trị, chăm sóc bệnh nhân, tích cực nghiên cứu khoa học, triển khai ứng dụng kỹ thuật điều trị mới. Xây dựng khoa Tai Mũi Họngphát triển đi lên cùng bệnh viện, tập thể Cán bộ viên chức khoa định hướng phát triển trên các mặt sau :

- Tiếp tục mở rộng và phát triển mạnh các kỹ thuật nội soi ít xâm lấn, tập trung đào tạo nguồn lực về chuyên môn kỹ thuật, ứng dụng kỹ thuật chuyên sâu trong các lĩnh vực phẫu thuật về tai, mũi xoang, đầu cổ, phẫu thuật nội soi.

- Đẩy mạnh công tác chăm sóc sức khỏe cộng đồng triển khai, các buổi khám bệnh chăm sóc sức khỏe cho người dân tại vùng sâu, vùng xa sẽ góp phần trong công tác an sinh xã hội của Đảng, góp phần quảng bá khoa, Bệnh viện Trung ương Thái Nguyên, đồng thời thực hiện các điều tra tầm soát bệnh lý.

- Mở rộng quan hệ chuyên môn trong nước và khu vực, chú trọng bồi dưỡng đào tạo cán bộ trẻ, nâng cao chất lượng công tác điều dưỡng chăm sóc người bệnh toàn diện trong điều trị, mở rộng quy mô về giường điều trị để đáp ứng nhu cầu khám, chữa bệnh của nhân dân trong khu vực là các nhiệm vụ then chốt trong thời gian tới khoa cần thực hiện.

6. The organizational structure

6.1. Development History

In July 1951, Thai Nguyen Central Hospital was established in Dong Hy - Thai Nguyen named Viet Bac Inter-Regional Hospital. At the beginning of its establishment, the professional activities and cell activities of the Department of Otorhinolaryngology were part of the activities of the Joint Department of Ophthalmology - Otorhinolaryngology - Maxillofacial Surgery.

In 1962, the Department of Otorhinolaryngology was separated from the Interspecialty Department. Party members of the Department of Otorhinolaryngology lived in the same cell as the Interspecialty Cell. In 2001, the ENT Party was separated and operated independently.

As a deep specialty within the surgical system, the Department of Otorhinolaryngology is responsible for examining, detecting and treating medical and surgical diseases of the ENT specialty to serve the people of Thai Nguyen Province and mountainous provinces. the North and neighboring provinces.
